Terminal 1 or Terminal 2 at MXP?

Central Milan has Area C, a charged congestion zone, and Area B, a wider low-emission zone. Traffic on the A8, the Tangenziali and the approach to Rho is heaviest at weekday peaks and on big fair days such as the Salone del Mobile, usually in April, and fashion weeks, usually in February and September. The Italian Grand Prix at Monza, usually in early September, fills roads north of the city.

What the Malpensa fare covers
Car, chauffeur, flight tracking and included waiting are in the fixed price, and gratuity plus any Area C charge on a drop in central Milan are added to the booking at actual cost. Child seats are free on request, useful on the lake roads.
Malpensa Express and other trains
The Malpensa Express runs from both terminals to Milano Cadorna in about 35-40 minutes and to Milano Centrale via Porta Garibaldi in around 50 minutes. Times are on Trenord. The train is good value for a light traveller heading to those stations. For hotels elsewhere in the city, the lakes, Rho with equipment, or late-night arrivals, a door-to-door car is simpler.
Lake Como, Lake Maggiore and Lugano
Many Malpensa arrivals skip the city altogether. Lake Como villas and hotels in Cernobbio, Tremezzo and Bellagio, the Lake Maggiore shore around Stresa, and Lugano across the Swiss border are all booked as fixed-fare intercity transfers. Bellagio takes longer, often 1 h 30 - 1 h 45, because of the lake road. Switzerland is in Schengen but outside the EU customs union, so customs checks at the border are possible.
Linate and private aviation
Milan's city airport, Linate, handles many domestic and European flights, and business jets often use the Linate private aviation terminal. Check your ticket, because the two airports are 60-80 minutes apart by road.
When to leave for a Malpensa departure
Arrive two hours before European flights and three hours before long-haul. Because the airport is a long way from the centre, a pickup from Brera or Porta Nuova is typically set 60-75 minutes before your target airport time, more during fairs and fashion weeks.

Turin, Genoa, the Alps and central Milan access
Malpensa's position west of Milan makes it a practical airport for more than the city. Turin is about 1 h 30 - 1 h 45 away by the A4, and Genoa roughly 1 h 45 - 2 h 15 by the A26 and A7, with Portofino a little beyond. Courmayeur, below Mont Blanc, is about 2 - 2 h 30, and St. Moritz can be reached in around 3 - 3 h 30 via Chiavenna and the Maloja Pass. Italian motorways charge tolls on most sections. In central Milan, the fashion streets of the Quadrilatero, such as Via Montenapoleone and Via della Spiga, have limited stopping and some pedestrian sections, so the chauffeur drops you at the nearest permitted point and helps with bags. Hotels in Brera and around the Duomo often have similar access rules, so give the exact hotel name when booking.
